***A parent, guardian or a parent /guardian designated, responsible adult shall deliver all medication to be administered by school personnel to the school nurse or other responsible person designated by the school nurse.

***Students are not permitted to carry any medication, except Inhalers or Epi-pens.

***Please use the absentee line to report your child’s absence.

***Only one physical is required upon entry to first grade. The preschool/kindergarten should have an annual physical due to their age.

***All immunizations, sent to the school, should have a signature from the practitioner on them. If your child receives updated immunizations, please provide them to the nurse for their health records.

***If your child has an injury requiring crutches or immobilization of an arm or leg, please call the nurse and I will provide assistance to the child. Any absences from PE and gym should have written documentation from your child's medical provider.
