Prescription Medication 

A written note from the physician and parent/guardian permission is required to administer prescription medication in school. Note: THE PRESCRIPTION LABEL IS NOT A WRITTEN ORDER. The note may be delivered by the parent/guardian, or faxed to the Health Office.

All medication must be delivered to the Health Office by an adult.

Over-the-counter (OTC) Medication

Parent/guardian permission is required to administer OTC medication at school.

Please review the district policy.  Forms are available in the last three pages for prescription,OTC, EpiPen, and inhaler.

Physical Exam

Please provide a copy to the Health Office as it occurs.
